The university hereby advises student and student acknowledges that by [participating in Activity] during the pandemic, the student may likely be exposed to COVID-19 or other risks that may result in illness, personal injury, or death, and the student understands and chooses to voluntarily [participate in Activity] and accept all risk to health that may result therefrom.
